Applications for all sessions and staff are available in
Online and Printable versions. 
Online links will take you to a form which will be
submitted directly to Camp Grassick. 
​Printable forms can be completed and mailed to
Elks Camp Grassick, PO Box F, Dawson, ND 58428 or scanned and emailed to [email protected].  If you need an application printed and mailed to you, please contact camp.
Late applications will be considered if space allows. 
​Application does not guarantee acceptance to any camp session.  A screening committee will meet in April to review all applications. ​Acceptance letters will be sent out in early May.

Please note that some applications require parts to be completed by parents, schools and therapists.  Make sure that all sections are completed by the deadline.

What you need:
  • Camp Application (To be filled out by parent/guardian)
  • School Report (To be filled out by special education teacher or classroom teacher)
  • Speech and Language Report (To be filled out by SLP if the student receives speech and language services)
  • Occupational and Physical Therapy Report (To be filled out by occupational and/or physical therapist if the student receives OT and/or PT services)
